Clutter Assessment Questionnaire
Please answer YES or NO to the following questions.
Your responses will help determine the scope of the clutter in your
home.
Structure
• Does your clutter block your exits?
• Can you fully open every exit door in your home?
• Can you fully open every interior room door to the fullest
width?
• Are you storing stuff in a bathtub or shower?
• Are all your kitchen household appliances working?
Household Functions
• Do you sleep in your bed?
• Do you cook in your kitchen?
• Can you use all your bathrooms in your home?
• Where (what room or area), do you eat your meals in?
• Have you ever tripped over your household clutter?
• Can you enter all your rooms and access all the windows
to open or shut them?
• Can you open and access all the closet doors either sliding
or walk in?
• How many rooms do you store your clothes in?
• Do you have a room you are unable to use or enter due to
clutter?
Pets and Rodents
• Do you have more than three dogs? (guideline within most
local animal control ordinances)
• Do you have more than five household cats?
• Do any of your pets have regular “accidents”?
• Do you have any non-domestic pets?
• Do you have any problem with ants, spiders, roaches, moths?
• Do you have any issues with pests such as bats, squirrels,
rats or mice within your home, including basement or attic?
Sanitation and Cleanliness
• Do you have visible mold or mildew on your walls, floors
or ceiling?
• Have you or someone else dusted your home within the last
month?
• Have you or someone else vacuumed or swept all of the floors
within your home within the last month?
• Have you or someone else cleaned and washed down your kitchen
and food preparation surfaces within the last week?
• Do you empty your household trash cans weekly to the exterior
trash receptacles?
What is Chronic Disorganization?
Chronic Disorganization is having a past history of disorganization
in which self-help efforts to change have failed, current quality
of life is affected by disorganization, and there is an expectation
of future disorganization.
